What is your goal? – Your Answer. This should only be one goal not multiple. You should have a separate journal for each goal.
Why do you want to hit this goal? – Your Answer
When do you plan to reach this goal? – Your Answer
Is this goal realistic?
Do you see any blockages/fears standing in the way of this goal?
Brainstorm you actions needed to achieve your goal:
List your actions needed to complete your goal. Be sure to list any issues or problems you need to solve to achieve goal success. This will be a long list. Make it a bullet list in your journal. Should be on a separate page or a couple of pages.

List the action your planning to take below your action column. Put the time you estimate the goal to be completed under the time estimated. Then check off when the goal is done under the done tab.

Goal Reflection:
Write a goal reflection. Write about your experience, what you’ve learned, any blockages you have experienced along the way, positive experiences you have experienced along the way, and how you feel now that you have accomplished this goal. This will at least be a page or more.
